Andrew was the fifth of 13 children born to Andrew Carmichael from Lislea, County Derry, Northern Ireland, and Isabella Johnstone from Middlebee, Dumfries-shire, Scotland.

Due to working on the farm and in their father's General Store, the children were home-schooled until about the age of 16. The parents wanted the best for their family and stressed education. Most of the boys graduated from Queens College - one son became a lawyer, another a clergyman and two, including Andrew, physicians.

After practicing in Ontario for almost a decade, Andrew married Minnie Medora Lockerbie and they settled in Parry Sound.

In 1909 Dr. Carmichael visited his brother John in Strathcona, Alberta, which was noted in the "Edmonton Bulletin." About 1910 the family with their six surviving children decided to move out west.
