Little Jack was born October 20, 1923, in Provo, Utah to Andrew Marinus Christensen and Florence Naillon Christensen, the fifth of their nine children. Little Jack passed away due to complications after choking on a foreign object (tack or nail). Provo doctors sent them to Pennsylvania for treatment for little Jack. Florence took him and rode the train back east. A telegram from Florence to Andrew stated the operation had been successful (published in the newspaper on March 11, 1925). However, little Jack passed away in Philadelphia a week later. Andrew was the informant on his death certificate, so he must have made the trip there within that week. They brought his little body back home on the train to be buried in Provo City Cemetery.

On January 8, 2014, at the beginning of my research, I found a record proving he did die in Pennsylvania, the Oliver H. Bair Funeral Records Indexes, 1920-1980, has a record for him with correct death date. I sent for a copy of little Jack Christensen's death certificate, hoping to clear up some mystery surrounding his death &/or learn why the family was in Pennsylvania at the time of his death. The certificate confirms name of parents, death date and place. However, birthdate of October 20, 1924, conflicts with headstone and cemetery records by one full year, which would have made him only 4 months 26 days old. Incorrect. Jack passed from this life at Jefferson Hospital in Philadelphia on March 18, 1925. Cause of death declared by Dr. Robert J. Patton: "Foreign body in lung and bronchitis." He was 17 months old.

Cemetery records also show he passed away in Philadelphia, Pennsylvania, but state he was buried the same day in Provo, Utah. Not possible. Funeral notice shows burial was March 23, 1925.
