Otha Monroe Leake is the son of Walter Scott and Margaret Isabelle Bellis Leake of Ray County, Missouri. He was one of 9 children. His siblings were George Washington, Gustavus Leonadus, Samuel Walter, Minnie Cordelia (Farris Miller), Mary Jane "Mollie" (Penniston), Florence Emma, Rufus Emmet, and Albert Adolphus.

The Leake family originally comes from England, settling in Goochland County, Virginia. This line of the family settled in Virginia before 1690, in Kentucky before 1790, and Missouri before 1840.

Otha died on the farm on which he was born at age 48, in Ray County, Missouri, from cancer of the throat. He did not marry or have children. He was a farmer.
