b 9 May 1863 - Shelby, Pottawattamie, IA (however Shelby spans both Pottawattamie and Shelby Counties
d 13 Oct 1863 - Camp Ft Douglas, Salt Lake CIty, Salt Lake Co, UT

Polly died on a trip made by her parents via wagon to sell goods to the soldiers sent to put down the "Mormon uprising" per researcher/descendants, Mari Nittler and Donna Kay.

This may be verified through US Nat. Cem. Interment Control Forms, 1918-1962, US Burial Register, Military Posts and National Cemeteries, 1862-1960 and Perkins family records.
