The annexed certificate of Board of Surgeons explains my utter incapacity to serve longer as an officer. I am respectfully, Yr. obt. Servt, R. P. Sweatt, Asst. Surgeon, P.A.C.S."
"Asst. Surgeon R. P. Sweatt of the conscript service having applied to me for a certificate upon which to ground an application for resignation, We certify that We have carefully examined this officer, and find him to be suffering with a severe form of "Chronic Impeligo [sic - impetigo]" and supervened upon chronic intermittent fever after a severe ptylism [excessive saliva]. It has resisted all remedial measures, and now from the severe excoriation produced by it, renders riding, walking, or any other exertion painful. We further declare our opinion to be that said officer cannot be relieved in any reasonable length of time, & that his resignation should be accepted.
