Betty's last name is listed as Cook on the death certificate but her father is listed as John Newton Coy. The obituary also gives her last name as Cook instead of Coy. The stone has Coy. The memorial reflects the last name Coy per Find A Grave rules.
The stone has the birth year and death year as 1945 but the death certificate and obit are 1944. It seems as if the stone was ordered at a later date since the year is incorrect.
