Azuba's parentage is unknown at this time. In 1790 there was an Isaac Cummings living in Hardwick and he may have been a relative.
Azuba was widowed in 1824. In 1830 it appears that she was living with her son Levi and his family, as was her widowed son Hosea B. Sprout. In 1840 she was living with Hosea.
In August of 1843 Azuba's son-in-law, Charles Toms, went on a visit from Jennings County IN back to New York. When he returned to IN he reported that Azuba was living with the Martin Townsends, and Hosea was living near the red school house. It could be that she was living with Townsends when she died.

On 14 Oct 2018, I learned that Azuba was the mother of Martin Townsend's wife, Susanna, who had died in Mar 1832. Susanna is buried in Schaub Cemetery in Attica NY, but she has a cenotaph in LaBelle Cemetery in Waukesha County WI where her husband Martin died.
