On the way back to West Virginia from Chicago, Dale was riding in the front seat passenger side of the car. The car was being driven by Harry Jarrell, a cousin, who also was killed in the accident. The plans were that they were going to stop at Dale's sister, Dody and her husband Charles Lego who lived in Cleveland, Ohio. The other passenger in the car was Charles Hawkins who was asleep lying in the back seat of the car. The breaks of the car had failed and they were unable to stop the car to pay the toll. Harry elected to hit a concrete barrier at the toll gate instead of crashing in the back of a car in line paying the toll. The impact threw Dale out of the car fracturing his cervical spine. Harry was crushed at the wheel. Harry and Dale were pronounced dead at the Angola Hospital in Indiana. Charles Hawkins survived the accident.
