On September 11, 1980 Whidbey SAR H-46 "Firewood One" crashed on Mt. Challenger in the North Cascades of Washington State on a rescue mission to Mt. Redoubt area. Killed were the pilot LCDR Dan Mahoney, CrewChief ADC Thomas Sanders, Swimmer AMS3 Richard Kubal, and Navy Nurse LT Patrick Kidgell and Corpsman HMC Roy Lewis. Copilot LT Rick Wall and Sheriff's Deputy Dustin Hurlburt survived though badly injured.
