His March 1893 declaration of intention was under the name: Wladyslaw A. Zakrzewski. A citizenship certificate granted on February 23, 1898 was also to Wladyslaw A. Zakrzewski. The baptism records for his four children (1892-1904) give his name as Ladislaus. His name normally appeared as Edward W. in newspaper items or deed records.

A date of birth and a place of birth (Kantyła, Kujawsko-Pomorskie, Poland) has been confirmed with a baptism record. This memorial originally gave a date of August 24, 1868--from the obituary in "The Frontier" of September 28, 1944 stated he was born in Poland on that date. His name was given as Ladislaus. His parents were not named. His mother, Marianna (Zagorska) Zakrzewski, died in 1870.

His marriage is not recorded at the Howard County courthouse but is found in the parish records for Our Lady of Mt. Carmel church at Chojnice (Paplin), Howard County:
Ladislaus Zakrzewski (son of Francisi [Zakrzewski] and Maryanny Zagorski natae Borus) to Justina Warminska (daughter of Casmiri and Anna) by Fr. Stuer on September 29, 1891.

The above 1891 marriage record is the only source of a name for his parents. The 1883 marriage record for his older sister, Antonina, named only her father--not her mother.

Only one source naming a town as his place of birth is an 1891 marriage license. The license recorded his place of birth as "Dombroski Germany." This location has not been verified.

Wladyslaw A. Zakrzewski obtained a homestead in Sherman County. Homestead certificate #11663 was issued for the northeast quarter of sec. 4, township 15 north, range 14 west (containing 158.50 acres) on December 27, 1898. He had filed the homestead application on March 14, 1893.
