Born Dec. 26, 1983, Millcreek, to John and Rebecca Cook. Married Annie E. Fayler Nov. 11, 1908, in the Salt Lake Temple, Church of Jesus Christ of latter-day Saints. She died in 1960. Served LDS mission to Washington state and British columbia in 1907. Former president Holladay Water Co. Employed by the Sugar House Lumber Co. Active member LDS Church.
Survivors; son, Clifton L. Salt Lake City; six grandchildren and 13 great-grandchildren.
