Mrs. Hazel Lewis, 67, Route 3, died Sunday at a local hospital.
Born in Omaha, she had resided 55 years in Council Bluffs and was a member of Westminster Presbyterian Church.
Survivors include: Husband, Albert; son, Albert Jr. of Los Angeles, Calif.; three daughter, Mrs. Dorothy O'Grady, Mrs. Patty Hytrek and Mrs. Norma Jean Olsen all of Council Bluffs; 12 grandchildren and two great-grandchildren.
Funeral services will be at 1:30 p.m. Wednesday at the Cutler Chapel. The Rev. John C. P. Gill, pastor of the Westminster Church, will officiate. Interment will be at Cedar Lawn Cemetery.
