Levi's obituary in the Free Press Standard, Carrollton, Ohio, Thursday, January 25, 1923.

Dr. Levi D. Allen, one among the oldest of East Ohio physicians, died in the home of his daughter, Mrs. Bessie Polen, at Amsterdam on Thursday morning last. For some time he had been in declining health, his final illness, however, dating from the Saturday before Christmas. Dr. Allen was in the eightieth year of his age, born at Petersburg, Carroll County, April 6, 1843. His education was largely acquired in the schools of the county, he taking up the practice of medicine in Amsterdam in 1869 and following the profession there throughout his life. He retired from active practice two years ago. As a physician his practice was quite extensive, reaching largely over the western portion of Jefferson and Carroll counties. In addition to his practice he owned a farm near Amsterdam and in active years when not engaged in his profession was frequently to be found at work upon this. For many years he served the village and community as a notary public, his long life thus ever marked with untiring and useful service to mankind. He was a member of the Methodist Church. His wife was Mary A. Armstrong, she dying more than 20 years ago.

Surviving are three sons and three daughters, Lafayette, Leroy and Al Allen, and Mrs. Bessie Polen of Amsterdam and Mrs. Lillie Gordon of Bergholz and Mrs. Laura Walker of Santa Clara, California. Two brothers, Jas. Allen of Union township and Jacob Allen of Alliance, are also living. Funeral service was conducted Saturday at 12:30 o'clock, with burial in Amsterdam Cemetery.
