The story of his death as told by his sister goes as such:
About a week before his death Harold came into the house and told his mother that he would be going to a new beautiful place soon. When his mother asked where, he stated he didn't know where he was going but was sure it was going to be a beautiful place. On the morning of Nov. 7th, Harold got up to go to work. As he started down the street his mother realized he had forgot his lunch. She hollered at him to come back and get it, but he looked at her and stated he didn't think he was going to need it. When he got to work, the elevator malfunctioned and he fell in the elevator shaft and was severely wounded. He died later at the hospital. Noone knows why he had the premonition that he was leaving to a better place, but he did.

The official report stated:

On November 7th, 1927 at approximately 9:00 AM Harold Kulow, 6806 Mansfield Avenue,[Detroit, Michigan] employed in the delivery department was in the act of stepping off elevator at first floor when in some manner car suddenly started up, pitching him down shaft into pit in subbasement. The car was being operated at the time by George Robb, 6054 Rohns Avenue. Kulow was taken to Receiving Hospital, where I learned he died later. The building electtician, Mr. Miller and myself checked over all interlocks and wiring and found them to functon OK at this time. It is recommended that the interlock on first floor gate be replaced so switch will not close by gravity if spring should break. Copied from report of Condon-Elevator Inspector
