Addison Blaklie Hoggard, infant daughter of Derek Hoggard and Kelsey Cook, passed away April 27, 2011.
Addison is survived by her aforementioned parents; her grandfather, James Albert Hoggard III, of Conway; great-grandparents, Susan Hoggard, Bur and BJ St. John and Nancy Lyon, all of Conway, and Mary Jo Washam, of Mayflower; aunts and uncles, Jason Hoggard, James Hoggard IV, Hope Quattlebaum, Sebrina Cook and Bailey Cook.
Addison is preceded in death by two grandmothers, Julie Ann Wall and Tammy Cook; and her great-grandfather, Roger Washam.
The funeral service for Addison will be 10 a.m., Saturday, April 30, 2011, in the Chapel of Roller-McNutt Funeral Home.
Interment will follow in Crestlawn Memorial Park.
