She often wrote epitaphs and poems for memorials. She wrote the one on their family marker for herself a couple months before she died. The one I posted in the transcription is from family notes, not directly from the marker although they should match.
Her physical description as given in family notes was "Height 5' 3", weight 120 lbs., habit (build) thin, fair complexion, light brown hair, blue eyes, and average health

My Last Poem
On life's troubled ocean, where perils assail,
My frail barque is driven, through many a gale;
Through long nights of darkness and sorrow and pain
For rest I am praying again and again,
To cross the dark waters, no longer to stay;
That carry me farther and farther away;
To reach a safe harbor, a haven of rest -
A home with my loved ones, a home with the blest.
What rocks to encounter on this frail barque of life;
What great waves of sorrow on this ocean of strife;
Could I but anchor in a haven at last,
Where sorrows are ended, and dangers are past."
