Orthella M. Hayes, 43, Rt. I, West Salem, was dead on arrival at 10 p.m. Friday in Richland Memorial Hospital.
Services will be in the Sivert Funeral Home with Rev. L. B. Howard presiding. Burial will be in Crest Haven Memorial Park Cemetery.
Mrs. Hayes was born Feb. 13, 1938 in Sumner, the daughter of Otto and Barbara (Cochran) Bellinger. She married Raymond E. (Jack) Hayes and he survives.
Other survivors include two sons, Jeffery L. and Dale A., both at home; six brothers, Kenneth, Olney; Charles, Mich.; Jack, Flat Rock; Gene, Lawrenceville; Tom, East Peoria; and Jerry, Dwight; two sisters, Mrs. Sandra Culter, Flat Rock; Martha Bellinger, Stedman, Mo.; and her parents, Mr. and Mrs. Otta Bellinger, Sumner.
