James was the son of Scottish immigrants James McCaw and Elizabeth Brough. He received his medical degree from the University of Edinburgh in 1792, and soon thereafter established himself in Richmond as a leading physician. Among his patients was noted legal scholar George Wythe, whom James attended after Wythe's poisoning in 1807.

He married Sarah "Sallie" Harris of Hampton in 1793; they had at least four children. He was re-married to Lucy Baugh by The Rev. Leonidas Rosser on 7 April 1845.

James was a hero of the Richmond Theater fire of 26 December 1811, being badly burned while assisting other patrons to escape. Famously, he worked with Gilbert Hunt, a free black man, to lower people from upstairs windows to safety.

Of his wives, Sally is buried near him in the plot he owned; Lucy is buried a short distance away. His marker was placed in 2019 through the efforts of the "Friends of Shockoe Hill Cemetery".
