Borm Sep 11 1922 in Manti, Sanpete, Utah to John D. & Katie Alfreda Jacobson Nielson. Married Carlos Jay Sudweeks June 5, 1940 in the Manti LDS Temple. He died 18 Jul 1978. Married George C. Provstgard, 30 Mar 1982 in the Jordan River LDS Temple. Employed at Mode O'Day as a seamstress for over 26 years. Was an officer of employees credit union and retired in November 1981. Held many leadership positions in the LDS Church. Member of DUP and American Business Women Assocoation. She is loved and will be missed by all.

Survivors: husband, Salt Lake City, Mrs. Daryl (Barbara) Frame, West Jordan Mrs. Ed (Karen) White, West Bountiful, Mrs. Bert (Mary) Little, Brent Jay Sudweeks, Mrs. Nicholas (Cindy) Curtis, all of Salt Lake City, four step-children, nine grandchildren, 30 step-grandchildren. Brothers and sister: Jay D. Nielson, Salt Lake City, Grant E Nielson, Van Nuys, Calif., Mrs. DeCoy (Lila) Larsen, Ephraim, Utah.

Funeral services Thursday 10:00 am 29th Ward chapel, 1130 West 500 North. Friends may call6:00-8:00 pm Larkin Mortuary, 260 East South Temple and Thursday at the Ward one hour prior to services. Internment Manti City Cemetery at 2:30 pm
