She was born to Wallace Albert Smith and Mary Freed Smith, she was united in marraige to George Wise on May 6, 1938 in Rapid City SD. She worked at ST John's Hospital as a housekeeper for many years before she retired. Hazel spent most of her life in and around Rapid City.
She is survived by two daughters Fern Rustad, of Ranchester WY and Mary Schreier of Kettle Falls WA, and one son Ray Wise of Colorado Springs CO. Two brothers Floyd Smith and Benny Smith both or Famingdale SD. One sister Betty Smith of Caputa SD.
She was preceded in death by her husband, her parents and five brothers and two sisters.
