Her eldest son, Barry Chase, also preceded her in death.
She is survived by her sons, Brian (Heather) Chase, Bernard Chase, Sr. and Bennett (Susan) Hayes; and daughter-in-law, Karen Chase.
Carolyn graduated from Woodruff High School in Peoria and formerly attended the Cathedral of St. Mary of the Immaculate Conception in Peoria.
She was the former owner and operator, from 1974 to 1979, of the Zodiac Lounge in Peoria. She was formerly employed at Caterpillar, Standard Oil and as an Avon representative.
She enjoyed sewing, making ceramics, playing cards and watching the Chicago Cubs.
