He was born Aug 9, 1949 in Taylorville, the son of Dutch and Aleane Damery Morthole, who preceded him in death.
Jack graduated from Taylorville High School in 1967 and earned a bachelor’s degree. He retired from Gateway Foundation, Springfield as a clinical supervisor in 2015.
He had a passion for flower gardening, photography, rock collecting and the Blues.
Surviving is his wife of 20 years, Deby Nation-Jacobs; daughter, Cassandra; three grandsons; sister, Jill; brother, James; several aunts, uncles, nieces, nephews and cousins
