He had been living and cared for with his daughter Augusta (Mrs. Stephen) Woodward, who was widowed on the 1910 census. Jacob P. Alberts was listed under Augusta, with no last name, so the transcription assumes his last name was Woodward too. Not so.
He was also living with Augusta in Todd County on the 1905 census.
This burial record came from the Lakeside Cemetery Burial records, which show his burial location as Sec. 1, Lot 18.
Someone else has entered him at Findagrave in a cemetery in Ramsey County, Minnesota. That findagrave # is 120427724
I don't think that is accurate, unless the family exhumed his body from Osakis Lakeside, and moved it to Ramsey County at a later date. But why would they leave a stone at Lakeside? There is no stone at Ramsey County.
