Elizabeth “Ann” Forker, 80, passed away on Sunday, September 30, 2018 at Advocate Condell Medical Center in Libertyville. She was born August 12, 1938 in Cape Girardeau, MO, was formerly of Webster Grove, MO and has been a Libertyville resident since 1973. She was a graduate of Drury College in Springfield, MO in 1960 and was a member of the Pi Beta Phi sorority and alumni association. Ann was the Libertyville Welcome Wagon hostess for many years, a volunteer for Libertyville Mainstreet and a former Libertyville volunteer of the Year. She was a member of the United Methodist Church and enjoyed her winter home in Marco Island, FL, bridge, reading and especially her family.
Surviving is her husband of 55 years, David Forker; 2 children, John (Kim) Forker and Sue Forker Smith and 5 grandchildren, Brandon & Morgan Forker and Nick, Katie and Elizabeth Smith.
A visitation will be from 4:00 p.m. until time of services at 6:30 p.m. Friday, October 5, 2018 at Burnett-Dane Funeral Home, Libertyville, IL. Interment will be at Lakeside Cemetery in Libertyville. Memorial contributions can be made to American Lung Association or Libertyville MainStreet.
