Mr. Ballard was born in Bartlesville, Oklahoma on September 8, 1950 to Samuel L. and Neomi Fay (Ashworth) Ballard. He grew up on a farm in the Ochelata area and graduated from high school at Ochelata. He was married to Melva Sue Bethel on October 12, 1974 in Bartlesville. He had been employed in the oil field for a number of years and also in building maintenance industry for a number of years. He then was employed as a sandwich artist at the Subway in Bartlesville until his retirement. He enjoyed gardening and nature and his grandchildren.
Survivors include his wife, Sue Ballard, four children, Sara Moore of Wylie, Texas, Symon Ballard , Samuel Ballard and Richard Harwick all of Bartlesville, three sisters, Mary Whaley, of Milan, Michigan, Lela Choate of Bartlesville and Judy Rex, two brothers, Leon Ballard and Jerry Larmore both of Houston, Texas and twelve grandchildren. He was preceded in death by his parents and stepmother, Maxine Ballard.
