Doing some research and found the notes for Ada Talyor 's mother Mary Ann Donaldson. Ada's death seems to be same as mother and document attached to the memorial says "and child" for Mary Ann Donaldson. I find a death record in Canada that seems to fit Ada That record has death 14 FEB 1892 in Oxford, Ontario, Canada and father listed as Edward C Taylor a turner. Her birth record says birth 25 Oct 1891 Oxford County . Oxford county fits the family
I have had no luck with a death record. I wonder if Mary Ann passed in childbirth and the baby was buried with her and unnamed.
