She was born April 12, 1913, in Chicago, daughter of Emil Andrew & Rose Caroline (Nienhaus) Johnson.
Hazel married Harry Melnick on August 3, 1936 in Cook County, IL, and was the mother of two children.
Mrs. Melnick was a retired secretary for the Chicago Typographical Union.
She was preceded in death by her parents; husband Harry; a daughter, Marlene (Charles) Fisher; and a sister Lorraine Barron.
