She was 76 yrs., 8 mos, 13 ds, born May 9, 1844 in New York. Her father was Moses R. Ballard and mother Eliza Beecher Ballard, both born in Mass. The date of death was Jan 22, 1921, after being treated from June 10, 1920 until the date of her death for senile dementia and atherosclerosis.
She was to be buried in the Nevada Cemetery alongside her husband in the Boyes family plot.
The informant for this report was Homer Boyes.
