She was born in Paynesville and one of 11 children born to Dr. Cary Randolph Bankhead and Amanda Ellen Errett. At the time of her death all of her siblings except one, Bess Bankhead, had predeceased her. She spent her girlhood in Paynesville and attended public school and the Pritchett Institute.

She married Clemence Griffith Smith of Clarksville in 1892 and they were the parents of four children. The bride and groom spent their first year of marriage on a farm near Clarksville then moved to St. Louis where her husband attended Washington University Medical School. After graduation the family moved to Cyrene where he practiced medicine for ten years until his death. Dr. Smith died from injuries he suffered when thrown from his horse when making a medical house call.

Nellie B. as she was affectionately known, and her daughters moved back to Clarksville where she spent the rest of her life. She taught music, was the Christian Church organist, city collector, librarian, and Democratic Committeewoman for many years.

At her death she was grandmother to five children and eleven great grandchildren.
