There's no error. Ruth was Harker's 3rd cousin and she was born "Ruth Spensley."

We went to bid goodbye to Mrs. Harker Spensley who is ill and not calculated to live long. Miss Roberts, Luella, was fishing in the morning. Mr. W., Miss Roberts and I started from Mineral Point to Dubuque about 2pm, leaving Mrs. Waller with her daughter, Mrs. Spensley. I enjoyed my visit with Mr. and Mrs. Spensley very much and invited them to pay us a visit should they ever come to England.
