Died while trying to save her younger sister, Matilda Eleanor Bagley in the Willamette River. Both drowned along with Mary Ann's little daughter Kate Josephine Leach who was aged 3 years.

"A week ago last Sunday the wagon of Samuel McCullough was upset while crossing the middle fork of the Willamette, by a wheel coming off. As a melancholy consequence, Mrs. McCullough her child and a Miss Bagly, aged about 12 years were drowned. Mr. McCullough narrowly escaping. The bodies of the mother and child have been found."

A burial memorial for the three individuals who died (Mary Ann, wife of Samuel McCullough; Matilda, Mary Ann's youngest sister; and Katie Josephine, Mary Ann's daughter) was made and had the following inscription:
"Sacred to the Memory of Matilda E. Bagley, Aged 12 years, 2 months, 8 days, Mary Ann McCullough, Aged 21 years, 10 months, 28 days, Kate Josephine Leach, Aged 3 years, 2 months, 23 days, Who were drowned April 9th, 1865."
