Survivors include daughter, Shannon (Tom) Zoltek of Fort Wayne; sister, Judy Johnson of Indianapolis; niece, Amy McKinney of Indianapolis; and nephew, Eric Johnson of Carpentersville, Ill. She was preceded in death by her husband, Robert E. Addison on Dec. 29, 1998.
