Ivy was born on August 1, 1937, in Ketchum, Oklahoma, to Harold Lee Forbis, Sr. and Jewel (Periman) Forbis. He married Juanima (Rhodd) on March 29, 1964, in Maud, Oklahoma. He was the Chief of Police in Wyandotte, Oklahoma for sixteen years. He was also a police officer for Spavinaw, Konawa, Maud, and Roff. He was a member of the Sacred Heart Catholic Church. He is survived by his wife, Juanima Forbis of the home; three sons, Dickie Forbis of Ketchum, Oklahoma, George Forbis of Picher, OK, and Kenny Forbis of Konawa, OK; six daughters, Vickie Cody of Coweta, OK, Jo Peck of Wagoner, OK, Sue Forbis also of Wagoner, OK, Sandy Forbis of Moundville, MO, Diana Cahoon of Konawa, OK, and Happy Mills of Pomona, KS; two brothers, Harold Lee Forbis, Jr. of Miami, OK, and Charles Lee Forbis of Joplin, MO; two sisters, Geneva Smoke of Ketchum, OK, and Woneda Moffett of Cleoria, OK; twenty-three grandchildren and eleven great grandchildren. He was preceded in death by his parents, Harold and Jewel Forbis; one sister, Dovie Lou Forbis; and one grandson, Billy Jack Coverdale. Rosary 7 p.m. today. Services 1 p.m. Saturday, Sacred Heart Catholic Church (Swearingen, Konawa).
