Forest Booth registered for the World War I draft on Sept. 12, 1918 in Hungington, Cabell Co., West Virginia. He was a laborer with the Chas Boldt Co. (In 1913, the Charles Boldt Co., which operated a glass bottle plant in Cincinnati, Ohio, began searching for a city where it could build a second factory. Attracted by Huntington's strong rail and river transportation network, the company purchased a tract of land on the city's South Side at West 5th Street.). He resided with his mother Rebecca in Huntington, WVA. He was described as tall, medium build with gray eyes and light hair.
